Our team will work with you, your family and those who support you to:
- Create a plan (design) for the service which will support you.
- The plan may include your home, your activities, who supports you.
- The plan will help you to have a good life, and to be involved in the area where you live.
- The plan will make sure your support keeps you healthy and safe.
