Skills for People are now working with the Outer West Primary Care Network in Newcastle upon Tyne, at six GP practices to support people with learning disabilities to access health services.
This will include supporting people to have their Annual Health Checks and helping them carry out Health Actions Plans. It will also include support for people to use other health services, such as routine screenings, over the coming months.
The GP Practices are:
- Throckley Primary Care Centre
- Parkway Medical Group
- Denton Park Medical Group
- Newburn Surgery
- Denton Turret Medical Centre
- Westerhope Medical Group
Sally and Joanna, from Skills for People, will be working as Learning Disability Social Prescribing Link Workers, alongside the Care Coordinators and Admin Teams, GPs, Practice Nurses, Health Care Assistants, and Practice Managers.
